The Truth About Anti-Aging Skincare: What Science Actually Says
You might think you know me. You might assume that what frustrates me most is skincare influencers spreading nonsense about sunscreen causing skin cancer—and yes, that truly enrages me. Or perhaps you think it's people claiming tanning is healthy—that makes my blood boil too. But here's something you might have picked up on: what I absolutely cannot stand, what actually pains me to consider happening to you, is spending money on things that are a complete waste. No benefit. No joy. Just empty promises.
In this age of constant visual stimulation, it's frighteningly easy to buy dumb stuff. And the elephant in the room? The skincare aisle. People spending hundreds of dollars a month on elaborate routines they see online. But is any of it actually doing anything to slow down, reverse, or prevent aging? Let me take you through what the science actually says works—and what is pure fluff.
Why Skin Ages in the First Place
Aging skin happens for multiple reasons. First, it's just a normal part of life—being on the planet longer naturally takes its toll. But a huge driver is cumulative sun exposure. And depending on your skin tone, this manifests differently.
If you have a paler skin type, you're more likely to see prominent wrinkles in sun-exposed areas, along with sallowness—that yellowing of the skin—prominent pores, and coarse wrinkling. If you have richly melanated skin, you don't show as profound wrinkling, but you do develop age spots, hyperpigmentation, uneven skin tone, and some coarse, dry texture.
The other big driver is lifestyle: smoking, excessive alcohol, poor diet, chronic stress. And for women going through menopause, the decline in estrogen has profound effects on skin aging. Women experience a lot more wrinkling during this phase. Interestingly, this menopausal wrinkling is different from sun-related wrinkling—it's a totally separate mechanism related to estrogen-deficient skin.
Here's where it gets fascinating: women with richly melanated skin tend to experience perimenopause and menopause earlier in life, have symptoms for longer and more intensely—yet they don't show the estrogen-deficient wrinkling to the same extent as those with paler skin. It seems their skin resists not only UV-induced wrinkling but also stands up better in the face of estrogen deficiency. Something deeper is going on beyond just melanin.
What Actually Works: The Evidence-Based Ingredients
Number One: Sunscreen
Without a doubt, no matter your skin tone, sunscreen is king. I don't care if it's chemical, mineral, or hybrid—daily use, not just intermittent, can actually reverse some existing signs of sun damage. For paler skin, this might look like fading age spots and a reduction in the formation of age-related wrinkles over time. It also reduces those pesky little pre-skin cancers. For richly melanated skin, it shows up as an overall reduction in uneven skin tone, discoloration, and dark spots. And yes, clinical studies on people with deeply pigmented skin confirm this.
Number Two: Topical Retinoids
Whether it's tretinoin, tazarotene, adapalene, or trifarotene—all of these have the ability to reverse visible signs of skin aging. They work by improving collagen production in the deeper layers of the skin, improving wrinkling. They also tackle discoloration at the surface. They slow down collagen destruction by suppressing those collagen-destroying enzymes. And they help with the maturation of the epidermis, so you don't get that buildup of dry, rough texture.
Most of these, aside from adapalene in the US, are prescription-only. But cosmetic forms of vitamin A like retinol and retinaldehyde can also be effective. They're not as well-studied as their prescription counterparts, but studies show they improve the appearance of aging—even if legally they can only claim to improve the appearance of things like fine lines.
Number Three: Alpha Hydroxy Acids
This one flies under the radar. Glycolic acid and ammonium lactate (the salt of lactic acid) are among the most robustly studied AHAs for reversing visible signs of aging. They exfoliate dry, rough texture and improve water content in the skin. They're humectants. Lactic acid, interestingly enough—especially at concentrations above 12%—does more than just smooth the top layer. With consistent use, it has been shown to improve the thickness of the deeper layers of skin too. You can find it in good old-fashioned AmLactin—an anti-aging product that truly flies under the radar.
The Hidden Champion: Niacinamide
This B vitamin is widely found in skincare products to the point where you can't walk through a drugstore or Sephora without landing on something containing it. You probably already own a product with it. First, it's an antioxidant, helping your skin handle damaging environmental exposures like sun and pollution. It improves moisture content and skin barrier function, which smooths out dehydration lines. It's excellent for dark spots because it disrupts the transfer of pigment from melanocytes to surrounding skin cells. It calms redness. And it's one of the few ingredients shown to reverse sallow skin—that yellowing related to glycation. I have a whole breakdown on skin sallowness, but niacinamide truly shines here.
The Vitamin C Question
Vitamin C has been shown to improve collagen production, signs of sun damage, and hyperpigmentation. But the form with the most evidence behind it is ascorbic acid—and not just any ascorbic acid. It has to be stable and at the right pH. The formulation with the most research support is ascorbic acid plus vitamin E plus ferulic acid—yes, that expensive serum you know. But there are other brands with similar formulations that likely get the job done.
Then there are vitamin C esters: magnesium ascorbyl phosphate, sodium ascorbyl phosphate, tetrahexyldecyl ascorbate, ascorbyl glucoside. Don't get it twisted—they likely have some benefits as antioxidants to cut down oxidative stress. They may help with dark spots to a certain extent. But we just don't have solid research on them. And importantly, they need to be converted to ascorbic acid in the skin to do what you want. It's not clear they actually achieve that.
What Might Not Be Worth Your Money
Peptides: The Jury is Still Out
There are so many peptides. Every other month, a new one gets released by some cosmetic manufacturer. They claim to improve collagen production, reverse signs of aging, tackle dark spots, and some even claim to act like Botox (argireline, for example). And yes, there is some research supporting these claims. But most of the mechanistic data comes from pre-clinical studies—cells in a dish. The actual clinical studies on humans show improvement in wrinkles but don't show changes in the biological parameters seen in the lab.
In other words, it's hard to know if they're doing much beyond moisturizing your skin—and we already know that moisturizing improves the look of wrinkles by hydration, space-filling, and softening lines. I like peptides. I use them. My skin gets along with them. Many have decent studies backing up that they reduce water loss and hydrate. But if you're debating whether to buy an expensive peptide serum that's not in your budget, please walk on by. It won't make or break your anti-aging goals.
Topical Estrogen: A Cautionary Note
For women with estrogen-deficient skin, topical estrogen may improve collagen and dermal thickness. Some dermatologists recommend it; others want more data. Studies don't suggest it causes major systemic side effects, but some research indicates it might have effects at distant sites. More research is needed.
But there are potential side effects: unwanted pigmentation, even localized hyperpigmentation in areas of application. It could bring out prominent dilated blood vessels. For women with richly melanated skin, the risk-to-benefit ratio is particularly unclear. As I mentioned, menopausal wrinkling doesn't affect them to the same degree. So boosting collagen production might not be necessary—and could potentially cause dark spots you don't want. I think it's reasonable to wait for more long-term safety research.
What Is Pure Hype: Growth Factors, Exosomes, and PDRN
Growth Factors
Growth factors get a lot of hype, but the science doesn't add up. Some worry they might accelerate skin cancer formation—a valid concern. But the fact that we've never seen this happen likely means they don't actually work the way growth factors in the body do. Instead, they probably work like many peptides—by improving water content. Nothing wrong with that, but not worth the premium price.
Exosomes and PDRN
These are all hype, with very little clinical science on actual human skin to support the claims. I've ranted about these ingredients before—they represent a wild west of product claims.
The Simple Truth: Keep It Simple, Keep It Consistent
At the end of the day, here's what works: sunscreen and moisturizer alone will help a lot. Wash your face at the end of the day to remove dirt, sebum, and cosmetic residues that can be irritating. That simple routine—cleanse, moisturize, sunscreen—does a lot. Add a retinoid on top, and you can effectively reverse visible signs of aging: improve wrinkles, hyperpigmentation, and barrier function.
That's it. You don't need a 10-step routine. Just keep it simple, keep it consistent. It's boring, but it's effective. It takes time for things to start working in the skin—but they do.
