The Hidden Compromise in Your Microneedling Routine
When it comes to treating hair loss with microneedling, two questions tower above all others: how often should you do it, and what needle length is best? We've covered the frequency elsewhere, but the length question requires a much deeper dive—one that balances therapeutic power with the gentle art of causing just enough controlled injury.
The correct needle length is always a compromise between two competing needs. On one hand, you need the needles long enough to maximize the therapeutic potential of your sessions. If you're combining microneedling with minoxidil—as most men do—you also want holes large enough to maximize absorption. On the other hand, you want to keep needles short enough to cause the minimum damage necessary. Longer lengths bring more redness, bleeding, swelling, and a higher risk of infection and serious complications. And yes, they hurt more.
To visualize this, consider a scientific study performed on pig skin. From left to right, the redness and bleeding increase dramatically as needle length goes from 0.5 mm all the way up to 2.5 mm. Dermatologists have proposed a classification: 0.25 to 0.5 mm causes mild injury, 1 to 1.5 mm is moderate, and 2 to 2.5 mm is deep.
A Quick Lesson in Skin Anatomy
To answer the question properly, we need a little background. The outermost layer of skin—what you see with your eyes—is the epidermis. This Greek word literally means "that which covers the skin." It has no blood vessels and is passively nourished from the underlying dermis. The word dermis, in Greek, means "skin"—and that's where the real action happens. Blood vessels pass through the dermis, and that's where your hair follicles are rooted.
You want the needles long enough to penetrate the epidermis and reach the dermis, but not so deep that they start traumatizing those follicles. Dermarollers on the market typically range from 0.25 mm to 2.5 mm. We know 0.25 mm is too short to sufficiently penetrate the epidermis. We also know that needles longer than 1.5 mm cause excessive damage, and no reputable studies use them. So the sweet spot for your scalp lies somewhere between 0.5 mm and 1.5 mm.
The Study That Changed Everything
Until recently, we had no direct test of this question. That changed a few months ago with the publication of a new study. Researchers recruited 45 men and women with androgenetic alopecia and treated them with six microneedling sessions over three months. They used an adjustable-length dermapen and randomly allocated participants to one of three lengths: 0.5 mm, 1 mm, or 1.5 mm. No minoxidil was used—this was standalone microneedling.
Three months after the last session, all three needle lengths resulted in regrowth. But the group treated with 0.5 mm depth had the best results, followed by 1 mm. The 1.5 mm group came a distant third. And as you'd expect, the 0.5 mm group also reported the least pain. A smaller needle length carried the day on all fronts.
Why the Dermaroller Changes Everything
This would seem to settle it: 0.5 mm dermarollers are the way to go, right? Not so fast. The electric dermapen used in the study pierces the skin vertically, meaning the full length of the needles penetrates. The downward motion comes from a small motor and is independent of the user. But the vast majority of people microneedling for hair loss use a dermaroller, and here two factors change the calculation.
First, due to the way the barrel of the dermaroller rolls over the skin, the needles enter at an angle, not vertically. This reduces effective penetration. Second, many users don't roll the barrel hard enough, further reducing actual penetration. When all is said and done, about 50 to 70 percent of the dermaroller's needle length will actually penetrate the skin.
So if you're like most people and use a dermaroller, a length of 1 mm is what you should aim for. This closely approximates the effective depth of a 0.5 mm dermapen. The underlying principle remains: match the effective penetration to the sweet spot of 0.5 mm, adjusting for the tool you're using. Your scalp will thank you with regrowth, less pain, and fewer complications—a rare win-win in the world of hair restoration.
