The second line goes from the nasal ala through the mid-pupil—this is the location of the arch. These two lines form the foundation of a mental map. From there, the injector calculates the dose and area of the forehead toxin, because the muscle that lifts the brow—the frontalis—usually ends at the temporal crest, but it can extend laterally. This is yet another variable. Every injector is thinking about these things, even if they are not drawn on your face.
The Treatments: A Symphony of Tools
Now, let us walk through the actual treatments. The first and most common is the neuromodulator—Botox and its cousins. When injecting this area, the easiest, most predictable, and safest zone is laterally, targeting the downward fibers of the orbicularis oculi muscle. But there are really three or four distinct areas to consider. In the diagram of the eye, you have the tarsal area, the septal area, and the body—the orbital part of the orbicularis oculi. The orbital part is responsible for a strong blink, a deep downward push. By manipulating these muscles, you can open the eye more and create a brow lift at the mid-arch. However, caution is needed to avoid dry eye. The muscle is complex, and there are four different zones where you can manipulate it to achieve a lift. This is a cursory evaluation, but it shows the depth of thinking. And if you do not get that perfect brow lift on the first try, stick with the same injector to find your pattern. Everyone's muscle is a little different—some are stronger, some weaker. Some will lift when you inject that area, others will not. It is a process of playing and manipulating until you find the unique pattern for your face.
Second, we have fillers. This includes both traditional fillers and collagen stimulators, which I use more often in this area. There are two key zones to fill: the brow and the temple. Very few people think of the temple, but consider this: a hollow temple creates a loss of the brow's arch. It leads to that "A-frame" shape, where the tail of the brow drops off. People try to lift their eyes, and the result is an unusual, unnatural arch. Fillers can restore volume and lift the brow. Also included in the realm of brow rejuvenation is hair growth. While microblading and tattooing are popular, using platelet-rich fibrin (PRF) or similar products can stimulate the growth of actual brow hair, contributing to the perfect shape.
The next treatment to consider is radiofrequency and other devices. Using heat to build and stimulate collagen can provide an immediate lift. People see it right away, and while that initial lift may fade after a couple of weeks, the long-term collagen growth in the forehead region is the true goal. This prevents lines from developing, which is important because the more toxin you use in the forehead, the more you risk dropping the brow and losing that perfect arch.
Finally, and this is crucial for everyone, PDO threads. I love smooth threads. If you have followed my approach, you know that to lift the brow, I place smooth threads above the brow. They build collagen and add a little volume. You can use twisted threads as well, especially if you want more volume. But the key is that they build collagen, so you do not have to use a neuromodulator in the danger zone at the temporal crest—that area where the frontalis ends.
