If you cannot answer that question with absolute certainty, you should not be injecting. This is the foundational truth that separates safe practice from guesswork. And yet, the most common debates in the industry revolve around angles—horizontal versus vertical—as if the direction of the needle alone could guarantee safety. But arteries do not live in a two-dimensional diagram. They live in three dimensions, and that changes everything.
Let me take you into the real anatomy of the lip. Most injectors have been taught that the superior labial artery lies at the wet-dry border or the vermilion border. Some papers argue for one, others for the other. But the truth is that the artery is not at either of those borders. It is 2 to 3 millimeters above—or more accurately, 2 to 3 millimeters behind—both of those structures. This is not a matter of opinion; it is a matter of three-dimensional anatomy. When you hear injectors claim that a horizontal injection is safer because it points away from the artery, they are missing the critical variable: depth. Depth is the defining factor. Depth is what makes you safe, not the angle of your entry.
Seeing the Lip in Cross-Section
To truly understand the lip, you must look at it in cross-section. This is the only way to grasp the concept of depth. The layers are clear: the dermis, then a very thin layer of hypodermis in the upper lip (where we often treat fine lines), then the most important structure—the orbicularis oris muscle. This muscle originates from the maxilla and mandible, branching into four sections that meet at the modiolus. It forms the strongest structural element of the mouth. Beneath the orbicularis oris lies the mucosa, and it is within this space that the superior labial artery is typically found. This is the vessel that most injectors fear, and rightly so.
The data is well-documented: the artery almost always sits at a depth of 4 to 5 millimeters. If you can visualize that depth in relation to the muscle and the borders, you gain a powerful reference point. The vermilion border itself is the junction where the orbicularis oris muscle inserts into the skin. This is where the muscle meets the two types of tissue: the white lip (skin and dermis) and the mucosa. That meeting point is your anchor. Everything in lip injection is relative to this insertion point.
The Insertion Point as Your Guide
When you inject, you can use this insertion point to navigate more accurately. If you insert the needle directly into the vermilion border, you are entering a highly sensitive area—more nerves, greater pain, and you are sliding directly into the muscle itself. Instead, if you understand the 3D anatomy, you can slide the needle purposefully underneath the muscle, into the volumetric part of the lip. The space between the entry point and the depth of the needle is surprisingly large on the average patient—so long as you are controlled and aware of where the vessels tend to lie. This is why depth matters more than angle. Whether you enter horizontally, vertically, or at any other angle, your safety is determined by how deep you go.
The Aspiration Debate: A Matter of Totality
Now, let me address a common question: Should you aspirate when doing lips? My personal practice is to aspirate nearly all the time, out of muscle memory. But it is absolutely possible to perform safe procedures without aspirating, provided you are using other safety measures. The key is to understand that it is not about any single technique. It is about the totality of all the safety measures you take. Aspiration is just one tool in a larger system.
