The Art of Controlled Injury: A Story of Microneedling
Imagine a treatment that works by purposefully creating tiny, controlled injuries in your skin. It sounds counterintuitive, doesn't it? Yet this is the core of microneedling—a procedure that uses devices with sharp, thin needle-like tips to puncture the skin at a very specific depth. This controlled form of skin injury is actually a good thing, because it triggers healing and remodeling pathways deep in the dermis, the deeper part of the skin. The result? A powerful stimulation of new, healthy collagen and elastin production. That's the essence of skin rejuvenation.
While microneedling originally gained fame for improving the appearance of wrinkles and fine lines through this collagen induction process, its popularity has grown as a treatment for a wide range of skin conditions where the root cause lies in the dermis. For example, it is remarkably effective for improving acne scars and other types of scars. It also helps with stretch marks, which are essentially a form of dermal atrophy—a loss of the supportive framework deeper in the skin caused by rapid stretching forces. Even hair loss disorders can benefit, as the stimulation of healing pathways brings nutritive and growth factors to the hair follicle, encouraging healthy regrowth. Some have even found success using microneedling for axillary hyperhidrosis—the medical term for excessive underarm sweating—likely because it can target the deeper layers where the sweat glands reside. And let's not forget hyperpigmentation issues like post-inflammatory hyperpigmentation; microneedling can improve that too.
But the benefits don't stop there. Microneedling can also be a way to enhance the penetration of topical medications or active skincare ingredients. You may have heard of people using microneedling to deliver the hair loss medication minoxidil more effectively. As you might know, minoxidil is a well-established treatment for certain types of hair loss, but not everyone responds well. The combination works because microneedling both stimulates healing pathways and, by bypassing the thick, waxy stratum corneum, allows minoxidil direct access to the dermis where the blood vessels are located.
Different Tools and Powerful Combinations
There are quite a few different microneedling devices, with various needle lengths that access different depths and are better suited to specific locations. And these devices can be combined with other skin-rejuvenating technologies. One powerful pairing is with radio frequency. Radio frequency is a type of energy that heats the deeper layers of the skin to stimulate healing pathways, and this energy extends below the tip of the microneedle. You get not only better collagen and elastin production but also a subtle skin tightening effect—highly desirable for many seeking rejuvenation. Furthermore, radio frequency microneedling has been shown to be very helpful for treating acne scars by exerting a thermal inhibitory effect on the sebaceous oil gland, reducing sebum (oil), which is a major player in acne.
Microneedling can also be combined with different LED therapies: blue light for acne treatment and red light for photobiomodulation to trigger collagen production, reduce inflammation, and promote healing. Paired together, they may offer even better results.
The Advantages of Microneedling
Microneedling is a remarkably safe procedure overall. It carries a very low risk of adverse effects. While there might be some discomfort, that depends on the location and needle depth. The risk of post-inflammatory hyperpigmentation is low. It is a cost-effective intervention for many skin issues, doesn't take much time to perform, can be done in the office without needing an operating room, and is suitable even for people with very sensitive skin or delicate areas like around the eyes. It has even been investigated as a treatment for rosacea, as it may stimulate anti-inflammatory pathways that reduce severity—though more research is needed.
When Microneedling Is Not for You
But microneedling is not all sunshine and rainbows. There are definite contraindications—situations where it is not a good idea. First, if you have an active acne breakout, do not drag this tool across your skin; it can aggravate existing breakouts. Second, if you are prone to herpes cold sores, especially during an active outbreak, microneedling can introduce viral particles into the tiny puncture wounds, leading to a serious skin infection requiring antiviral medication. You should also avoid microneedling if you have warts—even tiny flat warts on the face or beard area can spread extensively via the needles.
While microneedling is great for treating scars, if you have a history of forming keloid scars, it is not a good idea. If you have an active, poorly controlled chronic inflammatory skin condition like atopic dermatitis, eczema, or psoriasis, microneedling is probably not in your best interest. Psoriasis, in particular, exhibits the Koebner phenomenon (isomorphic response), where any form of skin injury—even just scratching—can elicit more psoriasis. Controlled wounding is clearly not advisable. Also, if you have underlying immunosuppression, poor wound healing, are on chemotherapy, or undergoing radiation therapy, you should avoid microneedling. Because it creates pinpoint bleeding in the deeper layers where blood vessels reside, if you have clotting problems, bruise easily, or are on anticoagulants (blood thinners), it may not be the best idea.
The Risks and Rare Complications
Even though microneedling is generally safe, it can go awry. The most common adverse effect is temporary redness and irritation, lasting only a few hours. You may also experience some peeling or flaking for a couple of days—that's just exfoliation. But rare complications do happen. It can trigger reactivation of the herpes virus, causing a more extensive and serious viral infection. Skin infections are possible: many people carry Staphylococcus bacteria in their nose, and if you microneedle in that area, you can introduce the bacteria and develop impetigo. It can also aggravate underlying acne. Hyperpigmentation is uncommon but possible, especially when microneedling is paired with radio frequency.
Applying certain topicals too soon after treatment can cause excessive irritation or even a foreign body granulomatous reaction if the product penetrates too deeply. This has been observed in people attempting at-home microneedling. In rare cases, you can develop tram tracking—little bumps that look like tram tracks—if the device is used with the wrong needle depth or if you press too firmly. Allergic reactions to the needle materials are also possible.
A Word of Caution on At-Home Devices
You can buy microneedling devices for home use. Many of these are probably only helpful for exfoliating the skin, as the needles don't penetrate to the appropriate depth to induce collagen and help with wrinkles or rejuvenation. However, you can buy devices that do reach those depths. I caution against using them yourself. Many adverse effects—especially infections—are seen with do-it-yourself treatments. Even if you clean the device perfectly, you are still at greater risk for infection, especially if you are prone to cold sores or carry staph bacteria. I've seen firsthand the development of granulomas from people doing microneedling at home. The risks are real, and while many people do it with no problem, I cannot recommend it.
Microneedling is a truly neat and helpful intervention. In the past five or six years alone, there have been many novel developments in how we use it to treat a variety of disorders. It's a treatment that offers profound benefits when done safely and professionally, and its future looks bright.
