The Art of the Brow: A Tale of Toxins and Timelessness
Picture this: you've lived a full life, laughed deeply, and frowned with concentration. Those expressions, the dynamic wrinkles born from movement, tell your story. But what if you could soften their tale, not to erase your history, but to give your face a moment of peaceful stillness? That's the promise of neuromodulators—a family of injectable tools that includes familiar names like Botox, Dysport, Xeomin, and the newcomer, Revance. Each one works on the same principle: they calm the muscles that create those expression lines, allowing the skin above to smooth out. The primary target is often the glabella—that space between your brows where the frown lines live. But the territory is vast. We can also address the crow's feet around the eyes, the forehead, and even the masseter muscles—those powerful jaw muscles that, when overactive, can cause grinding, bruxism, or give the face a wider, more square appearance. Weakening the masseter can slim the face. We can lift the downturned corners of the mouth by targeting the depressor anguli oris muscle. We can define the jawline and neck by injecting into the platysma, a sheet of muscle that can pull down the face. We can even soften the vertical lines around the lips or lift the brow. It's a delicate art, a conversation between the injector's knowledge and your unique anatomy.
Dysport: The Early Bird
In my own practice, my primary go-to is Dysport. Sold by Galderma, it holds a distinct advantage: it's a little less expensive than its famous cousin, Botox, and it kicks in sooner. You might feel the first hints of its effect within one to two days, with full results settling in by five to seven days. Botox, the originator, the brand name, the neuromodulator that started it all, takes a day or two longer to wake up—typically two to four days, with full effect by seven to ten days. And because it's the originator, it commands a higher price. Then there is Xeomin, a product from Merz. In my experience, it's an inferior option. Its absorption and results can be erratic. Sometimes it lasts the standard three months; sometimes it doesn't. It often requires more touch-ups. I don't use it in my office.
The Lasting Promise of Revance
But the story doesn't end there. A new player is entering the stage: Revance. This company initially hoped to create a topical neuromodulator—a cream you could apply along the crow's feet. The results were minimal. So they pivoted, developing an injectable form. And here's where it gets interesting. In their studies, they tested different doses—20, 40, and 60 units—in the glabella. They discovered that the 40-unit and 60-unit doses lasted up to six months. That's a breakthrough. Imagine only needing treatment twice a year instead of three or four times. That's their marketing promise: a six-month neuromodulator. But I must offer a caveat. A personal communication with Dr. Joseph, a respected researcher, suggests that if you inject Dysport at a double or triple dose, you might also achieve a five- to six-month duration. These studies aren't finalized, but they hint that Revance might not hold a monopoly on longevity. For Revance, the 20-unit dose didn't last as long, but there wasn't much difference between the 40 and 60 units, so they're recommending 40 units. The pricing will likely fall somewhere in between your current cost and less than double. It won't be cheap, but the convenience of fewer visits could be valuable.
Units, Dosage, and the Diffusion Myth
One of the most confusing aspects is the dosing. The units for Botox, Dysport, Xeomin, and Revance are all different. Botox was the originator, so everyone bases their units theoretically on Botox, even though they aren't directly equivalent. If you put 20 units of Botox into the glabella, a comparable dose of Dysport would be approximately 50 units. The ratio is roughly 2.5 Dysport units to 1 Botox unit, though it can vary between 2 and 3. Xeomin is close to Botox units, and Revance appears to be similar. This means Dysport is cheaper per unit, but you need more units to achieve the same effect. There's also a concern that Dysport migrates a little further than Botox. It's true—the protein complex associated with Dysport is slightly smaller, and theoretically, it can diffuse a bit more. But the active ingredient, the botulinum toxin type A, is the same in all of these products. In my own experience, with a skilled injector, diffusion is not a problem. All these products migrate approximately one centimeter from the injection site. The key is proper technique.
The Procedure and Aftercare: A Gentle Hand
Let me walk you through a typical treatment. Let's say a young female weightlifter comes to me. She uses her frown lines frequently. She's not fully resolved from a prior dose, so we'll treat her today. She has some forehead lines, but because the glabella injection diffuses upward, people with small foreheads often don't need a separate forehead treatment. I've drawn up 60 units of Dysport for her. My dosing in the glabella ranges from 40 to 70 units for strong muscles. I reconstitute 300 units of Dysport with 1.5 cc's of saline. Aftercare Rules
After the procedure, you must follow a few rules. No hot tubs, Jacuzzis, saunas, or working out. Anything that increases circulation and swelling can potentially spread the product. Don't lay yourself in a horseshoe position for a massage—that pressure can disperse the product. Don't manipulate the area, and don't wear a hat. All of these can move the product. How long before you can have a laser? It's best to wait three to seven days. Even though the product is absorbed in about four hours, I've seen a case where a laser performed 24 hours later dispersed the product a little. There's no correction for that. You can have chemical peels, fillers, and some laser treatments that don't cause significant swelling—like a superficial laser peel or a vessel treatment. But avoid anything that causes swelling, like a fractional laser resurfacing. Swelling is the culprit that disperses the product.
The Verdict: Which One for You?
So, to summarize: Botox kicks in a little slower but is the most established. Dysport kicks in sooner, lasts a little longer, and is a little less expensive. In my office, I use Dysport about 80% of the time versus Botox 20%. Xeomin is too erratic for me. Revance promises six-month duration. It's on its way for FDA approval, likely by late this year or early next year. Keep it on your radar. The studies have been positive, and it's a good stock, but I have no financial connection to the company—I simply believe in the science. The art of the brow is a nuanced one. It's about choosing the right tool for the right face, and it's about understanding that the goal isn't to freeze expression, but to soften the traces of time, leaving you looking like the best version of yourself.
