The Quest for a Better Minoxidil: Two Roads, One Destination
For decades, minoxidil has been a cornerstone of hair loss treatment—a molecule originally born as a blood pressure medication, only to reveal its unexpected gift for coaxing hair back to life. Yet, like all powerful tools, it comes with trade-offs. Oral minoxidil can cause side effects at higher doses, while topical versions are messy, inconsistent, and demand a level of discipline that many struggle to maintain. Now, two new approaches are emerging, both aiming to solve the same core problem: how to deliver minoxidil more effectively, with fewer unwanted effects. But they take entirely different paths—and understanding these paths is key to seeing whether they truly change the game.
The Sublingual Path: Bypassing the Liver
The first approach is sublingual minoxidil, placed under the tongue instead of swallowed. By absorbing directly through the oral mucosa into the bloodstream, it bypasses the liver's first-pass metabolism—a process that can alter how the drug behaves. The hope is to reduce side effects like palpitations and fluid retention while preserving efficacy. The version currently in trials is called SAM002, and a phase three placebo-controlled study is underway in Australia, testing 2.5 mg sublingual tablets twice daily in men with androgenetic alopecia. Recruitment is complete, with results expected in mid-2026.
An earlier 2025 study compared 5 mg of sublingual minoxidil to 5 mg of oral minoxidil over 24 weeks. The efficacy was similar, but the side effect profile was strikingly different. Palpitations occurred in 0% of the sublingual group versus 9% in the oral group. That is a meaningful difference—and it hints at the potential of this delivery method for those who are sensitive to the standard pill.
The Controlled-Release Path: Smoothing the Curve
The second approach is a controlled-release oral minoxidil, known as VDP301. Traditional oral minoxidil is immediate release: blood levels spike quickly and then fall. This peak is thought to be the culprit behind many side effects. VDP301 uses a matrix designed to release the drug slowly over an extended period, creating a smoother, steadier concentration in the bloodstream. The theory, according to a recent press release, is that hair growth may occur at levels as low as 1.6 ng per ml, while cardiovascular effects may start around 20 ng per ml. The idea is to stay within that therapeutic window—never overshooting into the danger zone.
Early, company-sponsored data show promising results: patients saw an increase of roughly 30 non-vellus hairs per square centimeter at six months, with over 80% reporting improvement and high satisfaction rates. No major cardiac side effects were reported. However, these results have not yet been peer-reviewed, and the company has not publicly shared the plasma concentration data that would verify their pharmacokinetic claims. Moreover, the dosing used in these studies—8.5 mg—is significantly higher than the typical low-dose oral minoxidil used in real-world practice, which is often 1 mg for women and 2.5 mg for men.
Side Effects: Not Eliminated, Just Shifted
When you compare the side effect profiles, the picture becomes nuanced. For VDP301 at 8.5 mg, peripheral edema rates were 5.3% to 6.3%, hypertrichosis (unwanted hair growth) was 3.5% to 6.3%, and discontinuation due to edema was about 1%, with no cardiac adverse events. In contrast, the largest study of low-dose immediate-release oral minoxidil (about 1,400 patients) showed hypertrichosis rates of 15%, fluid retention of 1.3%, peripheral edema of 0.3%, and a discontinuation rate of 1.7%—with typical doses of 1 mg for women and 2.5 mg for men. So while hypertrichosis appears lower with the controlled-release version, edema rates are actually higher. Side effects are still very much present; they are simply redistributed.
Importantly, minoxidil does not work the same in every person. It must be converted into its active form in the scalp by an enzyme called sulfotransferase, and not everyone has sufficient levels of this enzyme. Changing how minoxidil is delivered does not change this fundamental biological requirement. If you are a poor responder due to low enzyme activity, a different delivery system may not improve your outcomes. This is a point rarely emphasized in the excitement around new formulations.
What the Data Really Shows
Most of the data we have so far comes from the companies that manufacture these medications. That alone warrants a healthy dose of skepticism. When I examine the before-and-after images from these studies, I do not see a dramatic improvement over the results we already achieve with standard low-dose oral minoxidil in clinical practice. Compared to topical minoxidil, yes, these new forms are better—but compared to the way we prescribe oral minoxidil today, the difference is marginal. The biggest real-world issue with hair loss treatment is consistency. Oral minoxidil already improves adherence dramatically over topical versions. These newer delivery systems may offer incremental improvements, but they are unlikely to create a major shift.
And here is the most critical point: even with the best delivery mechanism, this is still just minoxidil. It stimulates hair follicles, but it does not prevent further loss. It does not address the hormonal drivers of the most common forms of hair loss. It is not a preventative treatment—and in my view, prevention is the most important type of treatment. These new formulations belong in what I call the "stimulation bucket." They are refinements, not replacements for the fundamentals of protecting your existing hair.
Cost and Practicality
These new medications will be expensive. The company behind the controlled-release version, Bioradix, raised about $770 million to support its drug discovery efforts—and that money will need to be recouped. Insurance is unlikely to cover them because they are considered cosmetic. In contrast, generic oral minoxidil is remarkably affordable: a year's supply of 2.5 mg daily costs around $200, or about $16 per month. For the vast majority of patients, the current option is completely sufficient. In my practice, we see side effects from low-dose oral minoxidil in about 1% of patients, and those are almost always managed by adjusting the dose. We rarely need to discontinue the medication entirely.
If a patient is doing well on the current form of oral minoxidil, there is no strong reason to switch. Expecting dramatically better hair growth from these new delivery systems will likely lead to disappointment. They are refinements—valuable refinements, perhaps, for the small subset of patients who are sensitive even to low doses. But they are not groundbreaking. They do not change the fundamentals of how we approach hair loss: a comprehensive plan that includes both stimulation and prevention, tailored to the individual.
The Bottom Line
These two new roads—sublingual and controlled-release—are both trying to solve the same problem: how to make minoxidil work better with fewer side effects. The sublingual version shows promise in reducing palpitations, while the controlled-release version aims to smooth out the drug's peaks and valleys. But both are still early in their journey, and the data must be viewed with caution. For most people, the current low-dose oral minoxidil—used wisely, with proper dosing and supervision—is already a powerful and well-tolerated tool. The future may bring better options, but for now, the fundamentals remain: consistency, prevention, and a realistic understanding of what minoxidil can and cannot do.
