The Foundation: The Hockey Stick Shape of Youth
Let’s begin with the orbicularis oris muscle. This is the most important structure for anyone treating lips, because it provides the underlying architecture. In a young, healthy lip, this muscle has a beautiful hockey stick shape. That shape is what we try to emulate, especially when creating feminine lips. It is preserved by connective tissue and by the volume of the lip itself. As we age, volume depletes, and the lip becomes longer, flatter. Our goal with treatment is to preserve or restore that crisp, youthful curve.
This desire leads many injectors to want to place product right up against that hockey stick tip—directly on the vermilion border. And that is exactly where the first problem begins. That border is the meeting point of all the tissues: the orbicularis oris muscle, the dermis, the hypodermis, all converge at that single line. If you inject directly onto it, you flood all those structures at once. But we only want to augment one compartment. Injecting directly on the vermilion border is the perfect recipe for filler migration. The filler spreads above and below the lip, softening the crisp signal that the brain reads as beauty. The result is a soft-focus, blurred lip that no longer looks natural.
The Invisible Safe Zone: Just Below the Meeting Point
So how do you add volume safely and cleanly? You must find the very small space just inferior to that insertion point. Think of the orbicularis oris plugging into the dermis like a wall. On the other side of that wall lie the hypodermis and the dermis above it. When you inject exactly into that tiny anterior compartment, you are using that wall as a natural barrier. The filler stays where it belongs, and the crisp shape is preserved. Your needle must be placed carefully, just a millimeter or so below the vermilion border, not on it. But this detail does more than prevent migration. It is also your shield against the greatest fear: vascular occlusion. The artery that supplies the lip—the superior labial artery—runs almost always on the underside of the orbicularis oris muscle. But in a real patient, with a smaller image and a moving target, it is easy to be two or three millimeters off. Many injectors who perform vertical injections through the white part of the lip are actually passing through the muscle and potentially through that artery. You’ve seen it on social media: a sudden gush of blood as a needle impales the superior labial artery. The difference between entering here (on the vermilion) and entering here (just below) is the difference between chaos and control.
Vertical vs. Horizontal: The Right Tool for the Lip
Vertical injections have their place, but not for every lip. Consider a young model with full, plump lips. There is plenty of space to insert a needle vertically without getting close to the muscle or artery. But what about a smaller lip? The muscle is roughly the same size, but the lip volume is less. A vertical injection in a small lip forces you to change your angle, pointing the needle directly toward the artery. It also requires more injections, leading to more swelling, bruising, and dissatisfaction. For small lips, vertical injections are more dangerous and less satisfying.
Horizontal injections, on the other hand, can be safer for thin lips—if you control the depth. The risk with horizontal injections is that the needle runs parallel to the artery, increasing the chance of cannulating it. Your primary defense is depth. You must picture yourself slipping into that small pocket of anterior compartment, no more than two to three millimeters deep. A simple test: when you lift the lip, if the whole lip moves, you have snagged the muscle and you are too deep. If you see only the mucosa lift and the shape of the needle, you are in the correct plane. Aspirate, check for easy needle movement, and then inject with a high-quality, soft product. This method dramatically reduces bruising, because most bleeding vessels lie deeper.
