The Myth of Unpredictable Anatomy
One of the most dangerous lessons an injector can learn is that anatomy is so variable you can never predict where a vessel lies. This belief cripples good judgment. If human bodies were truly that chaotic, we wouldn’t recognize each other as human. The truth is we all share a deep, predictable architecture, with only occasional variations. The key is to think probabilistically—not asking, “Where has this artery been on a random cadaver?” but instead, “What is the likely position here, and how does depth change the risk?” Over a career, this kind of thinking shaves off danger, one injection at a time.
Every safety method you already know—pulse checks, ultrasound, surface anatomy, aspiration, correct needle or cannula placement—still matters. But let’s layer in a deeper understanding of risk by exploring the most dangerous parts of the face and the mechanics that make them hazardous.
The Deep Danger Points
Start by asking yourself: What would be the most dangerous possible injection? Then do the exact opposite. To answer that, look not only at the vessel in the area but also at where it comes from—and which vital structures it connects to.
Near the Brain and Eye
The supratrochlear artery emerges from behind the eye and connects directly to the ophthalmic artery, which feeds into the brain. Blocking it is catastrophic. Deep injections on the bone here—right at the glabella—carry the highest risk. Next is the supraorbital artery, which shares the same origin and risk profile but is slightly smaller. Deep, bony injections in this region are the most hazardous you can perform.
The Midface and Jaw
The infraorbital artery runs through the maxilla and becomes part of the maxillary artery, which supplies the entire midface. Inject deeply with a needle near this foramen, and a blockage could cause not only superficial cheek necrosis but also damage to the nasopharynx or soft palate. The mental foramen releases the mental artery; occlusion here can affect the chin and lip, and if the filler reaches the alveolar artery, even the teeth. Though rare, a deep injection at this site is a serious gamble.
The biggest artery in the face is the facial artery, and its most predictable deep location is near the mandibular notch. A high-volume, deep injection right on that notch would be among the worst possible. When treating the jawline, it’s often safer to use a cannula, staying superficial in the fat and crossing this zone with extra care.
When You Run Parallel to an Artery
Beyond depth, the angle of your approach creates another class of danger. In several areas of the face, you may find yourself injecting parallel to a major vessel. This alignment makes it disturbingly easy to cannulate—slide inside the artery—and then inject filler directly toward the brain or eye.
The Nose: A High-Risk Parallel
The dorsal nasal arteries run just lateral to the nasal dorsum, parallel to the angle many injectors use with a cannula. If you slide inside that vessel, the product can flow straight toward the eye and brain. Safer approaches include needle injections at the periosteum, or using a cannula with vigorous movement, aspiration, compression, and ultrasound to verify you’re not inside the vessel. Every step reduces risk bit by bit.
The Nasolabial Fold
A cannula running parallel to the facial artery—common when treating the nasolabial fold—also raises the chance of arterial entry. This is one reason the deep periosteal injection technique gained popularity: at that depth, you’re beneath the artery, so even if you impale it, filler escapes underneath rather than pouring inside the vessel.
The Lips: Most Common Occlusion Site
Under the orbicularis oris muscle lies the superior labial artery, running parallel to the needle’s insertion point. Depth is everything here. Newer injectors often lack the fine resolution needed for millimeter-level accuracy. If you are two to three millimeters deeper than you think, you could be inside the vessel. Always perform a depth check and aspirate before each injection. If you cannot clearly see the needle’s shape through the tissue, you may be too deep.
Nasal Tip Lift
Injecting from the tip of the nose down to the maxilla and then back up to lift the tip is effective but dangerous. The injection runs parallel to the columellar artery, which sits at an intermediate depth—above cartilage and fat but below the dermis. Splitting your volume into smaller, deeper injections reduces the risk of a high-pressure blockage, even though occlusions here are rare.
The Glabellar Frown Line
The supratrochlear artery runs superficially under the crease you treat with filler. Injecting small volumes of very soft product, staying superficial while aspirating, and compressing the foramen during injection can limit risk to only the superficial blood supply. This approach makes the procedure safe enough for deep static lines.
Pressure Necrosis: The Silent Squeeze
Not all complications come from named vessels. Capillaries can be crushed by high-pressure injections, leading to a pressure sore–style necrosis. This happens mostly in areas with thick dermis or minimal potential space to expand: the glabella, the tip of the nose, and the chin.
Before injecting the nasal tip, squeeze it to gauge how much room exists between cartilage and skin. A very firm nose will barely yield—it has no room to expand. Injecting a large volume there creates dangerously high pressure. The same goes for the chin: if the tissue feels tight and adhered to the bone, instead of soft and expandable, large volumes can cause superficial blanching. In men, this may even lead to localized beard hair loss. The worst case is a superficial ulcer that may take weeks to heal and can leave a permanent scar.
The glabella is exceptionally sensitive. Many fillers are contraindicated there primarily because of this pressure necrosis risk. If you inject a firm filler directly into that thick tissue, the resulting blanching may never resolve—a white line that turns black as tissue dies. The solution is to use very soft fillers in low volumes, which is typically all that is needed to treat the crease.
Knowledge Over Fear
