Why Does Dissolving Filler Sometimes Fail?
There is a question that comes across my desk nearly every day, whispered through direct messages and etched into the comments of countless posts: "I tried to dissolve my filler, and it didn't go away. What went wrong?"
You are not alone in wondering this. Many people find themselves staring in the mirror, frustrated that a treatment they hoped would restore their face has instead left them with lumps, migration, or a stubborn bump that refuses to vanish. Today, we will walk through the hidden reasons why dissolving filler can fail, and what you truly need to understand before you try again.
The Enzyme and Its Promise
Inside your body, there are five naturally occurring types of hyaluronidases—enzymes that break down hyaluronic acid. What we do in a clinic is introduce an exogenous version of this enzyme. There are two main types: a recombinant DNA version, such as Hyalnex, and animal-derived varieties from ovine or bovine sources, which are testicular in origin. Both are powerful tools designed to dissolve hyaluronic acid filler.
Yet the story is never that simple. Some say that certain fillers, like Vycross, need five or six times more enzyme. Others claim brand matters. Having worked with all kinds of hyaluronic acid fillers for many years, I can tell you that the differences are not always obvious. Location, migration, and the age of the filler play far larger roles than the brand name. Fillers can last for many years, even though studies speak of a one-year efficacy. That efficacy refers to the visible result, not the complete disappearance of the product. The filler can linger, drawing water and distorting the landscape of your face long after the initial treatment.
The Five Silent Killers of Dissolving
1. Fake Filler
The most common culprit—far more common than people think—is that the filler you received was never hyaluronic acid at all. When you cut costs, when you go to a place that offers uncannily low prices, or when you order product from unknown sources, you risk receiving bio-alchemic compounds, PMMA particles, or even silicone. These substances are cheap to produce, and they do not dissolve. Once injected, they become a permanent problem. Surgical removal is often the only option, and even that is a nightmare because the particles scatter, scar tissue forms, and the tissue becomes a tangled mess. The lesson: if the price is too good to be true, your filler may be too cheap to dissolve.
2. Expired or Improperly Stored Enzyme
The hyaluronidase itself is a delicate compound. In Asia and Europe, different brands exist, each with its own dosing and efficacy. Compounded versions are also available. But this enzyme must be kept cool, and it has an expiration date. Many clinics dislike spending hundreds, sometimes up to a thousand dollars every few months to replenish their stock if they don't use it often. Some practitioners are even afraid to use it, worried that it will strip away your natural hyaluronic acid and ruin you for life. So they let it sit, sometimes for two years or more. The enzyme loses its potency. You might be injected with a product that simply cannot work because it was never stored correctly. I have seen this happen with botulinum toxins as well—products that were genuine but arrived warm, their effectiveness long gone.
3. Incorrect Dose and Placement
Even if your filler is real and the enzyme is fresh, the skill of the injector matters enormously. Hyaluronidase is a dispersant by nature; it spreads. If the dose is too small, it may not reach all the filler. If the dose is too large, it can dissolve more than you intended, including some of your natural hyaluronic acid. (By the way, your body makes hyaluronic acid constantly. Oculoplastic surgeons use high doses routinely as a dispersant for numbing, and it does not permanently destroy your native tissue. In rare cases, there have been concerns, but for most people, the natural product replenishes quickly.) The layer where the enzyme is placed is also critical—too superficial, too deep, and the filler either stays or spreads to areas you want preserved. This is not an exact science. I have used hyaluronidase for many years, and even I sometimes dissolve a bit of filler I wanted to keep. The complex reality is that you can always add more filler back, but you cannot un-dissolve something that was accidentally removed.
4. The Filler Might Be Old, But That Bump Is Not Filler
Another common scenario: you tried to dissolve an under-eye filler that was placed three or four years ago. It didn't go away. Could it still be filler? Yes, filler can last for years. But just as often, what remains is not filler at all. Prolonged filler placement can cause herniation of the fat pads. The malar fat pads—the soft fatty mounds under your eyes—can become stretched or displaced. Over time, a demarcation line forms around the pads, and what you feel is actually your own tissue, not the product. Poor lymphatic drainage from old filler can also create swelling that takes months to resolve. Sometimes it never resolves without a lower blepharoplasty. The point is, if the lump does not dissolve after proper enzyme injection, you may be looking at a structural issue, not leftover filler.
5. The Complexity of the Under-Eye Area
The under-eye and lips are the two most common areas for dissolving, and they are also the most challenging. Nothing about this area is simple. When you send me a picture saying, "I dissolved my under-eye and it still looks the same," I immediately think about the hundreds of factors: the age of the filler, the type of enzyme used, the storage conditions, the dose, the layer, the possibility of fat herniation, the possibility of poor drainage, and the possibility that the filler was never real to begin with.

The next time you consider dissolving filler, ask the hard questions. What product was used? Was it from the manufacturer directly? Is it hyaluronic acid? What enzyme will you use, and how was it stored? What dose will be placed, and at what depth? And most importantly, have you allowed enough time for your own tissues to recover from the original filler? Sometimes the best approach is patience, lymphatic drainage, and a thoughtful consultation with someone who has seen the full spectrum of both triumphs and failures.
